Free Sports Physicals

GVMS & GVHS Families,

SAVE THE DATE!

If your student plans to play in any sport next school year, they will be required to have a sports physical. Grand River Health will be providing FREE sports physicals. Battlement Mesa will have them on July 13th and July 14th from 4-6pm. Rifle will have them on July 15th and July 16th from 4-6pm.  Please check the flyer for more information.

Important: A parent or legal guardian must be present to sign the required CHSAA paperwork, including the medical history form. No student may participate in sports without this completed documentation. The CHSAA forms can be found at grandriverhealth.org under Free Student Sports Readiness Exams.

Si su estudiante planea practicar algún deporte el proximo ano escolar, deberá someterse a un examen físico deportivo. Grand River Health ofrecerá exámenes físicos deportivos GRATUITOS. Battlement Mesa los realizará el 13 y 14 de julio de 4:00pm - 6:00pm. Rifle los realizará el 15 y 16 de julio de 4:00pm - 6:00pm. Para más información, consulte el folleto.

Importante: Un padre o tutor legal debe estar presente para firmar la documentación requerida de la CHSAA, incluido el formulario de historial médico. Ningún estudiante podrá participar en deportes sin esta documentación completa.
